I am aware of the digital copy of the EFHS student handbook and the EFISD Student Code of Conduct that is posted on the web site www.efisd.org and agree to comply with the policy within them.  I understand that the handbook contains information that my child and I may need during the school year and that all students will be held accountable for their behavior and will be subject to the disciplinary consequences outlined in the Student Code of Conduct.  (A paper copy is available for you to refer to in the principal's office.)

All students will be issued a Chromebook, and will be responsible for it if it is damaged, broken, lost, or stolen.

Display student artwork, photos, or other original work on the district's web site.

Permission to participate in the District's electronic communications system.  (This allows the student to use the district's computers.)

Permission for certain personal identifiable information about my child or a photograph to be electronically displayed and produced by the District.  (This allows the students to have their photos used in the yearbook, newspaper, and on the school website--ex:  A/AB Honor Roll, recognition of awards, etc.)

Random Drug Testing authorization. (Must have approval to participate in UIL activities or to drive on campus.)

Release of Student Information  (Examples of information that may be released are listed in Release of Student Information at www.efisd.org.)

We, the parent/guardian, understand that emergency medications may be provided, as part of the Emergency Protocol, by the District.  medications on the emergency protocol may include the name brand or equivalent of the following:  Epinephrine, Albuterol 90 mcg inhaler, and Oxygen.  We understand that students who have previously been diagnosed with life-threatening allergies are required to provide their own prescriptions.

I have read and understand the Transportation Policy and Procedures.  (www.efisd.org)

Federal law requires that the District release to military recruiters and institution of higher education, upon request, the name, address, and phone number of secondary school students enrolled in the District, unless the parent or eligible student directs the district or to release information to these types of requesters without prior written consent.

I give permission for my child to be transported off campus by district transportation to attend any school-sponsored field trip or extracurricular event.
